How do I enroll in the MIRAPEX ER Door-to-Door program?
It’s easy to enroll in the program. All you need is a valid prescription for MIRAPEX ER along with the completed enrollment form. You can easily enroll online, or call 1-855-900-0786. Either way, we’ll contact your doctor to fill your prescription through Door-to-Door. You can also mail the enrollment form along with your prescription. You can visit our website at www.mirapexer.com to learn more about the program and download the enrollment form. Prescriptions and enrollment forms can be mailed to:

In accordance with Board of Pharmacy Regulations, only a physician may submit a prescription through fax, phone, or ePrescribe. Patients may mail original, hard copy prescriptions to Eagle Pharmacy. Eagle Pharmacy will fill medications upon receipt unless otherwise noted on the prescription to hold/profile by the patient or physician.

Who is Eagle Pharmacy?
Eagle Pharmacy is the trusted pharmacy partner for the MIRAPEX ER Door-to-Door program. Eagle Pharmacy is a retail pharmacy staffed by registered pharmacists and certified pharmacy technicians. The Eagle Pharmacy hours of operation are Monday through Friday, 8:00 AM – 10:00 PM EST.

How do I know if I am eligible for this program?
All patients are eligible for the MIRAPEX ER Door-to-Door program and there are no eligibility requirements. All patients with a valid prescription for MIRAPEX ER are eligible for the Door-to-Door program.

What if I’m already taking MIRAPEX ER but getting my medicine at a retail pharmacy? Am I eligible to enroll in Door-to-Door?
You are eligible to enroll in the MIRAPEX ER Door-to-Door program whether you are currently taking MIRAPEX ER or have been prescribed MIRAPEX ER for the first time. You simply need to send your prescription along with the enrollment form to complete your enrollment in the program.

How much will I pay for my MIRAPEX ER prescription?
You will never pay more than $30 for a one-month supply (30 tablets).

How is this program different than traditional co-pay card or similar savings programs?
Unlike traditional co-pay card programs or savings programs, all patients are eligible to enroll in the MIRAPEX ER Door-to-Door program and are guaranteed to pay no more than $30 for a one-month supply (30 tablets) regardless of insurance coverage. Even Medicare Part D and Medicaid patients are eligible to participate in this program.

Will I receive my MIRAPEX ER through the mail? How much is shipping?
A one-month supply of MIRAPEX ER will be mailed to you by Eagle Pharmacy, our MIRAPEX ER Door-to-Door pharmacy partner. Your prescription will be shipped free of charge directly to your home within 5-9 days. Expedited shipping is available for a nominal fee.

Can I use my commercial insurance coverage if I enroll in the MIRAPEX ER Door-to-Door program?
Patients without insurance restrictions and co-pays of $115 or less will pay a $10
co-pay for a one-month supply of MIRAPEX ER. Patients whose co-pay is less than $10 will continue to pay their usual co-pay. For patients whose co-pay is between $115 and $135, they will pay that amount minus the maximum savings benefit of $105 per month. No one will pay more than $30. Patients with insurance restrictions or co-pays of more than $135 will not be processed through insurance and will pay $30 for a one-month supply (30 tablets) of MIRAPEX ER.

What if I have Medicare Part D or Medicaid insurance? Can I still enroll in this program? How much will I pay?
Yes, patients with Medicare Part D and Medicaid are eligible to enroll in the MIRAPEX ER Door-to-Door program. If you have Medicare Part D and 1) do not have insurance restrictions and 2) your co-pay is $30 or less,
you will pay your co-pay amount (not to exceed $30). If you have insurance restrictions or a co-pay of more than $30, your order will not be processed through insurance and you will pay $30 for a one-month supply (30 tablets) of MIRAPEX ER.

Patients with Medicaid cannot use their insurance coverage to cover the medication in this program and instead will pay the cash price of $30 for a one-month supply (30 tablets).

Am I eligible if I don’t have any insurance? How much will my MIRAPEX ER cost?
Yes, even patients with no insurance are eligible for this program. You will simply pay the cash price of $30 for a one-month supply (30 tablets) and standard shipping is free. This is a significant savings over what you would pay for brand-name MIRAPEX ER at a retail pharmacy.

What happens once I send in the enrollment form and my prescription? How do I check the status of my order?
Once we receive your completed enrollment form and prescription information, we will process your order and your medication will be shipped directly to your home within 5-9 days as long as no order processing issues arise. You can check the status of your order by calling 1-855-900-0786 or by emailing BI@eaglepharmacy.com (email is monitored Monday through Friday 8:00 AM – 05:00 PM EST, excluding holidays).

How do I order a prescription refill?
You can refill your prescriptions by going online to www.mirapexerd2d.com and clicking on the Already Enrolled? SIGN IN button, or you can call us at 1-855-900-0786. Customer Support representatives are available Monday through Friday 8:00 AM – 10:00 PM EST, excluding holidays.

You can also elect to refill your medication through the Automated Refill program. Each month, your medication will automatically be shipped directly to your home and the credit card on file will be charged prior to shipping. Patients are responsible for all shipments and co-pays while enrolled in this optional service. You may opt in or out at any time by speaking with one of our Customer Support representatives at 1-855-900-0786.

MIRAPEX ER is a prescription medicine used to treat the signs and symptoms of Parkinson’s disease.

What is MIRAPEX?

MIRAPEX is a prescription medicine used to treat:

  • the signs and symptoms of Parkinson’s disease.

What are the possible side effects of both MIRAPEX ER and MIRAPEX?

Both MIRAPEX ER and MIRAPEX may cause serious side effects, including:

  • falling asleep during normal daily activities.

    • MIRAPEX ER / MIRAPEX may cause you to fall asleep while you are doing daily activities such as driving, talking with other people, or eating.
      • Some people taking the medicine in MIRAPEX ER / MIRAPEX have had car accidents because they fell asleep while driving.
      • Some people did not feel sleepy before they fell asleep while driving. You could fall asleep without any warning.

      Tell your doctor right away if you fall asleep while you are doing activities such as talking, eating, driving, or if you feel sleepier than normal for you. Don’t drive or do other dangerous activity until you know how MIRAPEX ER / MIRAPEX affects you.

  • low blood pressure when you sit or stand up quickly. After you have been sitting or lying down, stand up slowly until you know how MIRAPEX ER / MIRAPEX affects you. This may help reduce the following symptoms:

    • dizziness
    • nausea
    • fainting
    • sweating

    Sit and stand up slowly after you have been sitting or lying down.

  • unusual urges. Some people who take certain medicines to treat Parkinson’s disease, including MIRAPEX ER / MIRAPEX, have reported problems, such as gambling, compulsive eating, compulsive buying, and increased sex drive.

    If you or your family members notice that you are developing unusual urges or behaviors, talk to your doctor.

  • hallucinations and other psychotic-like behavior (seeing visions, hearing sounds or feeling sensations that are not real, confusion, excessive suspicion, aggressive behavior, agitation, delusional beliefs and disorganized thinking). Your chance of having hallucinations and other psychotic-like behavior is higher if you are age 65 or older.

    If you have hallucinations or other psychotic-like changes, talk with your doctor right away.

  • uncontrolled sudden movements (dyskinesia). If you have new dyskinesia, or your existing dyskinesia gets worse, tell your doctor.

  • skin cancer (melanoma). Some people with Parkinson’s disease may have a higher chance of having melanoma than people who do not have Parkinson’s disease. It is not known if the chance of having melanoma is higher because of the medicines to treat Parkinson’s disease, or from the Parkinson’s disease. People who take MIRAPEX ER / MIRAPEX should have regular skin examinations to check for melanoma.

Avoid alcohol when taking MIRAPEX ER or MIRAPEX. It can increase your chances of having serious side effects.

The most common side effects in people taking MIRAPEX ER for Parkinson’s disease are:

  • nausea and vomiting
  • constipation
  • dizziness
  • fatigue
  • dry mouth
  • swelling of the feet and ankles
  • headache
  • weight loss (anorexia)

The most common side effects in people taking MIRAPEX for Parkinson’s disease are:

  • nausea
  • dizziness
  • insomnia
  • constipation
  • muscle weakness
  • abnormal dreams
  • confusion
  • memory problems (amnesia)
  • urinating more often than usual

These are not all the possible side effects you could experience. Tell your doctor if you have any side effect that bothers you.

Before taking MIRAPEX ER or MIRAPEX, tell your doctor about all your medical conditions and about all the medicines you take, including prescription and over-the-counter medicines, vitamins, and herbal supplements.
